Women’s Health Hormone Self-Assessment

If you’re concerned about symptoms that may be related to hormonal imbalances, the Hormone Self Assessment Questionnaire is a useful tool. The brief questionnaire asks for general information about your personal and family medical history, your current medications, and the degree to which you are experiencing complaints ranging from insomnia and depression to hot flashes, vaginal dryness, and fatigue.

College Pharmacy Hormone Consultations

After you’ve filled out the Women’s Health Hormone Self-Assessment, call or visit College Pharmacy to make an appointment for a low-cost consultation with one of our advising pharmacists that specialize in Biologically Identical Hormone Replacement Therapy (BHRT).
Each 30 minute consult costs $45.00 and includes a follow-up visit. Together, you can review your self-assessment questionnaire and discuss your options. The pharmacist may suggest appropriate saliva or blood tests to check your current hormone levels. You will then receive a personalized recommendation for BHRT, which can be given to you or faxed directly to your healthcare practitioner. (A prescription from a licensed healthcare practitioner is required for all hormone prescriptions.)

Women: Hormone Levels & Lab Tests

We recommend you have the following hormone levels tested before starting hormone replacement therapy:

  • Progesterone
  • Estradiol & Estrone
  • DHEA (Sulfate)
  • Free & Total Testosterone
  • Cortisol
  • Vitamin D3 (25 Hydroxy)
  • Thyroid (TSH, T3, T4)

These hormone levels can be tested by completing a blood or saliva test.

  • Blood testing can be ordered through your doctor. You can also use an independent laboratory that offers services direct to patients.
